Home Support Services

Are you unable to go out to receive care and services? Do you have difficulty performing your daily tasks on your own? With the help of those closest to you and the resources available in your community, our Home Support Program can help you continue to take care of yourself.  Our top priority is to help you maintain your functional independence so that you may stay in your home for as long as possible. To this end, an interdisciplinary team will work with you, your loved ones and our partners to reduce risk factors and ensure your safety.

Who are home support services aimed at?

You may be eligible for in-home care and services, regardless of your age, if you are:

  • Experiencing a loss of independence
  • Disabled
  • Convalescing
  • Living with a chronic disease
  • In end-of-life care

Every situation is assessed and analyzed on a case-by-case basis together with the individual concerned.

Composition of the Interdisciplinary Team

The Home Support Program consists of an interdisciplinary team that will assess your needs. The number of professionals involved will vary based on your specific needs.

  • Nurses
  • Licensed practical nurses
  • Doctors
  • Health and social services assistants
  • Social workers
  • Occupational therapists
  • Physiotherapists
  • Nutritionists

Care and services available

  • Nursing care (wound care, post-operative care and follow-ups, specimen collection, etc.)
  • Rehabilitation services (physiotherapy and occupational therapy)
  • Psychosocial services
  • Medical care
  • Clinical nutrition
  • Home support services (help with personal hygiene, medication supervision, etc.)
  • Day centres: group activities for seniors (prevents isolation, promotes independence, etc.)
  • Referral to social economy enterprises, community organizations and other partners 
  • Support for informal caregivers
  • Palliative care
  • Grief support
  • Referral service to residential facilities

How do I go about getting home support services?

Do you or a loved one think you may be eligible for home support services?

Contact your CLSC  to make a request:

Request trajectory

  1. Person requesting home support services:
    • Person with a disability
    • Informal caregiver
    • Health and Social Services Network Professional (doctor, nurse, physiotherapist, etc.)
    • Intervener working with a community organization
  2. Access Portal: welcome, assessment & guidance
    • The network professional contacts the user or their representative, analyzes the request, and validates eligibility.
  3. Eligible request
    • Case manager is assigned, according to the level of priority
    • Detailed evaluation and interpretation of needs
    • Creation of a personalized intervention plan
  4. Services provided
    • Home care visits
    • Care and services
    • Follow-up and ongoing re-evaluation of needs

If your request for home support is turned down and deemed ineligible, you will be redirected to the appropriate resource, where applicable.
