Cardiometabolic Risk Program

Who is this program intended for?

The program is intended for individuals living with prediabetes, diabetes and high blood pressure. The goal of the program is to learn about healthy lifestyle habits and to have support while making certain lifestyle changes.

Services

One-on-one and group meetings:

  • Lifestyle modification program (risk factors, medication)
  • Blood glucose assessment and monitoring by a nurse clinician
  • Blood pressure improvement program

Low Back Pain Management Program

Who is this program intended for?

This program is available, upon medical referral, to adults living with low back pain. The goal is to develop the skills needed to self-manage pain.

Services

One-on-one meetings offered by an interdisciplinary team. The goal of these meetings is to: 

  • Ensure early detection of those at risk
  • Prevent complications and chronic conditions amongst those targeted and encourage personal independence

CNESST and SAAQ cases are excluded.

To participate in this program, you must have a medical referral and must meet certain criteria. Your physician must fill out the referral form for the CIUSSS de l’Ouest-de-l’Île-de-Montréal’s chronic illnesses program.
