Access to services and contact information

Please have the following documents (if you already have them) with you when you come to the CLSC: 

• All diagnostic reports 
• Other relevant documents (professional assessment reports:   occupational therapist, speech therapist, dietician, etc.)
• Québec health insurance card 

Are you (or your child) already receiving services from the West Montreal Readaptation Centre (WMRC) or one of the CLSCs in our territory? 

If so, please contact the person you (or your child) are working most closely with. They will be able to answer your questions and guide you.
