What is the Specialized Nurse Practitioner (SNP) Clinic?
The SNP clinic offers complementary front-line services, providing additional support to referred patients. It supplements existing services.
Who can access these services?
Services are available upon referral to people of all ages without a family physician who require:
- preventive and health promotion care;
- care for common or chronic health problems;
- care to manage minor emergencies, in collaboration with the Home Care Services Department (SAD);
- proximity care and community interventions in collaboration with local organizations, particularly for vulnerable clienteles, including people who have recently arrived, people experiencing homelessness, people without a health insurance card, and members of Indigenous communities.
How to access these services?
Currently services are available by referral only. Patients without a family physician can be directed to the SNP clinic via:
- Redirection from the Emergency Department: healthcare Professionals can direct P4 and P5 patients without a family physician to the clinic using the Québec Medical Appointment Scheduler;
- The GAP service: for certain users without a family physician;
- Internal referrals: Caseworkers with the various CIUSSS departments car refer individuals without a family physician using existing patient trajectories.
