Before your hospital stay
Go to the Admissions Office to complete paperwork in preparation for your admission. If you are unable to do so because of your physical condition, a loved one may go on your behalf.
Please have the following with you:
- Valid provincial health insurance card
- Hospital card (if you do not have one, the Admissions Office will give you one)
- Information about your private insurance, if applicable (to cover the cost of a private or semi-private room)
- List of medications including over-the-counter medications and natural products
- Other relevant documents from the referring doctor
