In the event of an incident or an accident, with or without time loss, or if damage to personal property occurs, employees must fill out a report and have it signed by their manager. This report fulfills a legal obligation.
Declaration, investigation and analysis of an accident or a dangerous situation
When filling out the document above, you must check one of the following two options depending on the situation that led to the incident or accident: Workplace Accident or Dangerous Situation. Please refer to the definitions and information below to determine the most appropriate option.
Work accident (that is, an incident that has occurred): A sudden and unforeseen event, attributable to any cause, which happens to a person, arising out of or in the course of their work and resulting in a physical or psychological employment-related injury (Act respecting industrial accidents and occupational diseases). It can also be an unwanted event that causes an injury, material losses, or a loss in productivity.
Dangerous situation (that is, a situation in which an accident could occur): Feared or undesirable event that could have happened (recent past) or that has the potential to cause injury or affect the health and safety of individuals. Events that result in property damage without injury are also included in this definition.
- This declaration is intended to provide employees and managers with a system an information and management system for identifying, correcting, and managing all significant risks to health, safety, or physical and psychological integrity of staff with as promptly as possible.
- It should be noted that the more frequently at-risk situations of a given type occur, the higher the probability of an accident.
Did you know that...?
- Declaring a workplace accident is not a means of punishing or reprimanding an employee. Declarations are a means of protecting you. When a workplace accident is declared, the employer must identify the cause and develop solutions to ensure that the event does not recur.
- The Act respecting occupational health and safety states that the employee and the employer must take part in detecting, declaring, and searching for solutions to eliminate any possible risks of workplace accidents.
- The PHPD’s prevention team reads all declarations of workplace accidents and at-risk situations reported at our CIUSSS.

These appendices allow the manager to fully understand how the workplace accident occurred. The manager can then take immediate action to remedy the situation in order to prevent future accidents.
