The CIUSSS de l’Ouest-de-l’Île-de-Montréal is kicking off its 2 for 1 campaign to remind users of the importance of identification and the use of two person-specific identifiers.
What’s the point of two identifiers?
Users must be correctly identified before receiving any treatment or service. This step, which is carried out in partnership with you and your loved ones, helps prevent errors and ensures that the right person is getting the right treatment.
How does it work?
Before receiving any treatment or service, two different pieces of information are used to confirm your identity. These identifiers can be:
- Your full name (first and last name)
- Your date of birth
- Your health insurance number
- A recent photo
- Your home address
You will be asked for two identifiers in the following situations:
- Upon arrival/leaving a resource or when being transferred
- Before medication is administered
- When first interacting with a healthcare professional
- At the beginning of every follow-up or provision of services
