Background, Disclosure, and Reunions Services


Are you adopted? Was your child adopted? Are you related to someone who is adopted?

In Québec, institutions that provide protection and readaptation services to youth in difficulty and their families are also responsible for background, disclosure, and reunion services. 

All requests must be sent to the Background, Disclosure and Reunion Services that is responsible for the region where the person was adopted. 


Who do we serve ?

Our CIUSSS is responsible for background, disclosure and reunion services for:

  • The anglophone community on the Island of Montréal 
  • Montréal’s Jewish community

Our services are available in both French and English, as set forth in the Act Respecting Health Services and Social Services.

How to access our services?

  1. Complete the application
  2. Make a photocopy of two valid pieces of identification (for example, a health insurance card, driver’s licence, passport, a birth certificate). 
  3. Send the documents:
